What is a PracID?

A PracID is an Alberta Health Practitioner identification number.

This number is required for you to refer your patients for x-rays, or to any other medical practitioner.

How do I get my PracID?

A PracID is obtained directly from Alberta Health:

• To request your PracID application form, contact Alberta Health at 780-422-1522.
• You will need to send them a copy of your practice permit. It can be downloaded from your members' centre.
