Submit a complaint
After reviewing our
complaints process, if you feel ready to submit a complaint against a chiropractor, you must complete the form below. The College of Chiropractors of Alberta will accept complaints only via our reporting form, as it ensures that we begin with the information we require.
CCOA complaint process and reporting form
When you have completed the
complaint reporting form, including the attached authorization of release of information, sign and date them. Attach copies of any documentation to support your complaint. Do not submit originals of your supporting documents. Ensure you keep a copy of your submission for your records.

When the CCOA receives your complaint, the Complaints Director will review it and determine how to proceed based on options outlined in the
Health Professions Act.
Important

We are unable to accept verbal, faxed, or emailed complaints, as we require original signatures.
- A complaint form that does not contain your first and last name, phone number, a couriering address, and original signature cannot be processed.
- Anonymous complaints are not accepted.
